Our organization is required by law to maintain the privacy of your personal information and to provide you with a notice that describes our legal duties and privacy practices. Help for Housing is committed to keeping your personal information confidential and we will follow the privacy practices described in this notice. We believe that protecting your privacy is one of our most important responsibilities. Because you have entrusted us to protect your privacy, we want to provide you with a complete explanation of how your personal information may be used and to whom it may be disclosed. We will explain the use and disclosure of your information when required or allowed by law and when required for your services. We will also explain your rights to access and control how your personal information is used.
Personal information is about you. It includes information that identifies who you are and may include your contact information and the services you receive. We may document demographics about you, such as your race, ethnicity, sex at birth, preferred gender, and primary language spoken. We may also document physical or mental health conditions that affect your service provision and access to services. Each time you have contact with our organization for services, documentation is created. This information is used to coordinate your services, maintain a legal document for your services, and provide data to those funding our services, including the federal government.
We will communicate with you via all means including mailings, telephone, voice mail, text, and email. Communications are made to provide you with appointment reminders, discuss referrals, provide you with education, obtain updates from you, and to share information on services.
We will only share information about you without your authorization when required by law, including law enforcement, court order, judicial or administrative proceedings, or other legal requirements, when required by public health authorities such as local, state and federal agencies., during audits by oversight organizations, government authorities involvement victims of abuse, neglect or violence, coroners or medical examiners, disaster relief efforts, or other areas as provided by law.
We will not disclose your information without written authorization by you except for reasons named above. If you do authorize our organization to use or disclose your information, you can revoke that authorization at any time.
You have the right to request your information. We ask that you submit these requests in writing. Reasonable requests for access to and copies of your information and documentation may be submitted in writing to the case manager assisting you in services. We may charge a reasonable fee to cover the cost of printing and copying the information.
If you feel the information, we have about you is incorrect or incomplete, you may ask us to amend the information by submitting a request and reason in writing to the case manager assisting you with services. We may deny your request for an amendment, and if this occurs, you will be notified of the reason.
You have the right to make a reasonable request for a list of disclosures we have made of information. You must submit this request in writing and must state a time period, which may not be longer than six years from the date of the request. Your request should indicate what form you wish to receive the information, such as electronic or on paper.
We are required by law to notify you of a breach within the parameters of privacy laws.
